Forms instead of Tickets Reports

From now on, the Ticket Report List will be re-named as the Forms List. When creating a form, it will be possible to choose either a Ticket Form or an Acceptance Form. Additionally, an "i" icon will be added, suggesting you when a given form type should be selected. An additional column will also be added to the list of forms, indicating its type.
